Please note FCS is not a crisis service and it can take up to three business days for the FCS provider to receive this and respond to you.

This is an initial enquiry only, the FCS provider who receives this will contact you and discuss your eligibility to receive a service.

Service Provider Selected

An FCS service provider can make initial contact with a client even if they haven’t provided prior consent for the enquiry. However it is always best practice to let the client know about the enquiry and to get their consent for the enquiry to be made on their behalf. It will be up to the client if they wish to continue receiving support from FCS , as FCS is a voluntary service.

*The personal information provided by you in this form is collected by DCJ for the purpose of facilitating your enquiry to an appropriate service provider. The information provided in this form, is provided voluntarily. If you do not wish to provide some of the information asked for in the form, this will not inhibit the process of an enquiry being submitted, but may result in having to provide more detailed information when you are contacted by a service provider. The information you provide in this form will be held by DCJ and will be made available to a service provider for the purpose of contacting you and determining if they can provide you with support. The information will be held by DCJ no longer than four weeks after your enquiry is sent to the service provider after you are contacted by a service provider, after which time it will be deleted. A request can be made after this time to access and amend your personal information through the relevant service provider. DCJ will collect personal information from service providers to evaluate the effectiveness of the services provided to you and the success of the Family Connect and Support Program. The information held by DCJ may also be used for research. You have the right to access the information DCJ holds about you and to correct it, if the information is inaccurate. Please contact the Family Connect and Support program team if you would like to access or correct your personal information.
